Emergency Medical Form

Lakota Emergency Medical Authorization Form

This form is required for band camp and all band activities, including games, competitions and trips.
This form must be notarized and have a current photograph of your student attached (one of the small school photos would be perfect).
Please write the student's full name on the back of the photo and attach it to the lower right corner of the emergency medical form.
Please turn in the emergency medical form by July 31 at the latest.

Medication Permit Forms

Students are not permitted to keep any over-the-counter medications or prescriptions with them at band camp or other band functions.
Any student needing prescription medication or over-the-counter medication during band camp, trips, or other band activities, needs to have a medication permit signed by the parent and physician on file with the band nurse otherwise they will not be able to receive any medication while on band activities.
An exception to this policy is made regarding inhalers and EpiPen's. The band nurse needs to have a medication form filled out by you and your physician stating this. They also need to tell the band nurse each time they use their inhaler/EpiPen.
This is Lakota policy and state law.

This form is to be used for prescription medications and over the counter medications not listed on the Lakota East Bands Medication Permit.
This first form needs to be signed by a parent, filled in and signed by a physician and then turned in to the band nurse with the medication in the original container at band camp registration.

This form is for Band Use Only!
It is partially filled out with over-the-counter medications that the band nurse will have on hand at band camp, games, competitions, and trips.
If you want your student to be able to take these medications while with the band, fill in the top portion and have your physician fill in and sign the middle section.
Return the completed form to the band nurse at band camp registration (you do not need to bring the listed medications with your student; the band nurse will have them on hand).

Marching Band Registration Form 2011

The form states the Lakota Board Of Education's Substance Abuse Policy as well as the consequences.
This form needs to be signed by both students and parents.
You should print two copies of this form. Return the signed one to the band director and retain the other copy for your records.

This is a list of Upbeat Club Activities through the school year that require volunteers.
Filling out this form does NOT obligate you to volunteer for any particular activity. This just provides the Upbeat Club with a list of potential helpers.
