Lakota Emergency Medical Authorization Form

• This form is required for band camp and all band activities, including games, competitions and trips.
• This form must have a current photograph of your student attached (one of the small school photos would be perfect).
• Please write the student’s full name on the back of the photo and attach it to the lower right corner of the emergency medical form.
• Please turn in the emergency medical form on Monday,  July 30 2012.

Lakota School Medication Permit

• Only for overnight trips.
• This form is to be used for prescription medications and over the counter medications.
• This form needs to be signed by a parent, filled in and signed by a physician.
